যথাবিহীত সম্মান প্রদর্শনপূর্বক আমি নিম্ন স্বাক্ষরকারী বিনয়ের সাথে জানাচ্ছি যে গত ২৫.০৯.২০২১ তারিখে আপনার ব্যাংকে আমার হিসাব নং ১১৫-২০২০-১৩১ অনুকূলে একটি ডেবিট কার্ডের জন্য আবেদন করি। এরপর ০১.১০.২০২১ সালে আমার আবেদনের প্রেক্ষিতে আমার বর্তমান ঠিকানায় প্রিমিয়ার ব্যাংকের ইপিজেড শাখা হতে একটি চিঠি আসে যাতে ডেবিট কার্ড প্রাপ্তির বিষয়টি রয়েছে। কিন্তু দুর্ভাগ্যবশতঃ চিঠি খুলে আমি কোনো ডেবিট কার্ড পাইনি।

উপর্যুক্ত পরিস্থিতি বিবেচনায় মহোদয়ের নিকট আমার বিনীত আরজ এই যে, আপনি যথাযথ ব্যবস্থা গ্রহণ করে যথাসম্ভব দ্রুততম সময়ে আমার হিসাব নং এর অনুকূলে একটি ডেবিট কার্ড প্রদান করে আমাকে বাধিত করবেন।
